The Ensign Group, Inc. (Nasdaq: ENSG) announced the acquisition of operations at three skilled nursing facilities, adding 232 licensed beds to its portfolio. The acquisitions include Ironwood Rehabilitation and Care Center (80 beds) and Lakeside Rehabilitation and Care Center (100 beds), both located in Coeur d’Alene, Idaho, and Toluca Lake Transitional Care (52 beds) in North Hollywood, California. These transactions were effective June 1, 2025.
The Idaho facilities will operate under a long-term triple net master lease with a third-party landlord, while the California facility's real estate is expected to be acquired by a subsidiary of Standard Bearer Healthcare REIT, Ensign’s real estate arm, pending regulatory approvals.
These additions follow a broader acquisition agreement announced in December 2024, involving a total of eight facilities from Providence Home and Community Care. The Toluca Lake facility is the first to transition under that deal.
With these changes, Ensign now operates 347 healthcare operations in 17 states, including 44 senior living operations. The company, through its subsidiaries, owns 144 real estate assets tied to its healthcare services.
Executives cited operational alignment and expansion in key markets like Idaho and California as drivers behind the acquisitions. No financial terms were disclosed, and there was no update on future earnings impact or integration costs.
Ensign also reiterated its strategy of acquiring both high-performing and underperforming facilities across the U.S. through leases or direct real estate ownership, signaling continued expansion activity in the skilled nursing and senior living sectors.
