Fresenius Medical Care (NYSE:FMS) shares are trading down 4.07% at $21.69 today after a third-quarter report and narrowed outlook. Adjusted operating income of 545 million euros beat consensus, but the company cut the top end of its 2026 operating income guidance to flat to a mid-single-digit decline. The stock is near the low end of its 52-week range of $20.02 to $26.42 on Volume well above average, even as the German listing recovered from early losses.

Fresenius Medical Care is a Bad Homburg, Germany-based provider of kidney care services and dialysis products, with a Frankfurt listing alongside its New York shares. Its Care Delivery segment, about 80% of revenue, operates roughly 3,500 to 3,600 outpatient dialysis clinics treating around 290,000 patients, and its Care Enablement segment manufactures dialysis machines, dialyzers and other disposables. It reported roughly 19.6 billion euros of Revenue in fiscal 2025, about 72% from the United States.

Tedapa Reimbursement and Weaker U.S. Treatments Weigh on the Outlook

New Chief Executive Shervin Korangy sounded cautious, and management flagged a possible 300 million to 350 million euro Tedapa-related headwind in 2027, along with about 1% in CMS rate contributions for its book of business. The 2026 outlook now sits in the lower half of the prior range, with operating income growth between flat and a negative mid-single-digit percentage.

U.S. same-market treatments fell 1.1%, the third straight quarterly decline, against full-year expectations of about negative 0.9%, and China exit costs cut reported Net Income by 42%. Operating income excluding special items fell 6% at constant currency on Tedapa reimbursement headwinds and higher inflation.

Care Delivery and Enablement Show Underlying Resilience

Care Delivery revenue rose 2% organically and operating income rose 2% with a 14.7% margin, and excluding Tedapa underlying operating income rose 21%, helped by a transitory Medicare outlier reimbursement benefit. Value-based care revenue fell 9% on a contract restructuring, though operating income improved to a small profit from a loss.

Care Enablement revenue grew 5% and Earnings 4%, driven by the 5008X rollout and pricing. The 5008X is in 348 clinics, with 20% of the install base expected to be converted by year-end, and high-volume hemodiafiltration made up about 1.3% of treatments in the first nine months, accelerating in October.

Buyback Continues and the 2027 Outlook Arrives in February

Net Leverage is 2.6x, and the ongoing 1 billion euro buyback continues, with the first Tranche expected to be completed by mid-December. FDA audit findings are being addressed with corrective actions. The 2027 outlook is due in February, and analysts remain cautious, with a consensus target near $23.42.

February 2027 Guidance Is the Key Reference Point

With the shares near the low end of their 52-week range, the focus is on how management frames Tedapa and treatment volumes in the 2027 outlook. The 5008X rollout and buyback are the main offsets.
