A Quarter Built on Two Products

Glaukos Corporation (NYSE:GKOS) shares jumped in pre-market trading after a third-quarter report that cleared estimates on both revenue and profit. Net sales rose 58% from a year earlier, and the adjusted profit replaced the loss the market had pencilled in.

The composition of the beat is the more telling part. The iDose TR implant contributed about $76 million and Epioxa, a corneal cross-linking therapy for keratoconus, about $53 million. Together they account for roughly 61% of the quarter's sales. Epioxa made up nearly 90% of corneal health revenue of $59 million, which grew 154%. In U.S. glaucoma, iDose TR delivered close to two-thirds of the $118.6 million total, which grew 47%. International glaucoma sales of $33.4 million grew 13%.

Glaukos, based in Aliso Viejo, California, pioneered Micro-Invasive Glaucoma Surgery after its founding in 1998 and has been listed in New York since 2015. The quarter shows a company whose growth now rests on newer products rather than the stent franchise that built it.

What the Numbers Imply for Valuation

The raised 2026 range lifts the sales midpoint by about $90 million, to $780 million. At the prior close, the market capitalisation of $10.7 billion equalled roughly 13.7 times that midpoint. At the pre-market price, the multiple rises to about 16 times 2026 sales and about 12 times the $1 billion marker for 2027. Trailing earnings per share remain a loss of $0.86 and the forward price-to-earnings ratio sits near 234, so the shares are priced on the expectation that profits catch up with sales.

Margins point in that direction. Non-GAAP gross margin widened by about 400 basis points to roughly 88%, and non-GAAP net income was $28.5 million. Fourth-quarter operating expense growth is guided at 25% to 30%, which would put 2026 operating expenses near $630 million, up just over 30%. Spending will rise further in 2027 as disease awareness campaigns expand.

Management said the $1 billion figure allows for downside scenarios, including a restrictive Medicare coverage policy for iDose TR. A proposed local coverage determination is pending, and the regional contractors have until May 2027 to act. The market estimate for the average price target stands near $217.54, close to the indicated pre-market price.

Catalysts Still to Come

A Phase 4 trial found that iDose TR combined with cataract surgery lowered intraocular pressure by a mean of 11.1 mmHg, 3.6 mmHg more than cataract surgery alone. Phase 2 data for a Demodex blepharitis treatment and for a tyrosine kinase inhibitor programme are both due by year-end. FDA approval for iDose TRIO is targeted by the end of 2027, and a keratoconus screening device is slated to launch entering 2027.

What Investors Will Weigh Next

The quarter strengthens the case that Glaukos can scale its newer products, but the shares now trade above their prior 52-week high of $191.62 and the valuation leaves little room for disappointment. The Medicare coverage ruling, the pace of operating expense growth and the year-end trial readouts are the next tests of the $1 billion marker.
