Medical Properties Trust, Inc. (NYSE:MPT) reported third quarter 2026 Net Income of USD 0.34 per share, well above a consensus estimate of a loss of USD 0.01 per share and a swing from a USD 0.13 loss a year earlier. Net income attributable to common stockholders was USD 201.6 million versus a loss of USD 77.7 million. Total revenues were USD 250.1 million, up from USD 237.5 million but under the consensus estimate of USD 256.89 million.

Normalized funds from operations were USD 63.8 million, or USD 0.11 per share, compared with USD 77.2 million, or USD 0.13 per share, a year earlier. Chairman, President and Chief Executive Officer Edward K. Aldag, Jr. described the quarter as evidence of decisive steps to strengthen the Balance Sheet and create room to cut Debt further and pursue selective growth.

Asset Sales, Refinancing and Tenant Pressure

Two large items lifted the quarter result. A gain of USD 157.8 million on real estate sales and a USD 69.5 million benefit from debt refinancing lifted net income as well. The company sold its remaining interest in five Utah hospitals for about USD 174 million and its interest in the real estate of two Idaho hospitals to affiliates of Intermountain Health for about USD 371 million. It also completed a private offering of about USD 2.4 billion of secured notes, using proceeds to repay the 2026 notes and roughly 50% of the 2027 notes.

Offsetting this, real estate and other Impairment charges were USD 23.9 million, and interest expense rose to USD 153.1 million from USD 132.4 million. The company said its tenant HSA faces Revenue cycle management challenges that precluded payment of September rent, which cut normalized funds from operations by two cents. HSA recently engaged Ovation Healthcare to oversee operations across the portfolio and improve billings and collections.

Portfolio, Debt Profile and Stated Rent Figure

Total Assets were about USD 14.4 billion, including USD 8.3 billion of general acute facilities, USD 2.4 billion of behavioral health facilities and USD 1.6 billion of post-acute facilities. The portfolio held 365 properties and about 37,000 licensed beds leased to or mortgaged by 52 hospital operating companies across the United States and eight other countries. Cash and cash equivalents were USD 599.3 million.

Gross debt stood at USD 9.37 billion at quarter end, with 53.7% maturing in 2032, USD 383.3 million due in 2026 under the revolver and USD 865.4 million due in 2027. The company projects portfolio cash rents above USD 1.0 billion in 2027 even after removing rent from assets sold in the quarter, and that estimate uses conservative assumptions for cash basis tenants including HSA.

Signposts for the Fourth Quarter and 2027

Several concrete items are open. HSA rent is the first, since missed September rent trimmed normalized funds from operations by USD 0.02 and the Ovation Healthcare engagement targets billing and collections. The second is the debt schedule, with USD 383.3 million of revolver borrowings due in 2026 and USD 865.4 million in 2027.

The third is the cash rent figure of above USD 1.0 billion projected for 2027. The fourth is use of the USD 680 million of sale proceeds, which the company said is available for debt repayment, alongside the quarterly Dividend of USD 0.09 per share paid in October 2026.
